In home assessments to determine plan of care for a variety of impairments to promote to regain functional engagement in daily living activities.
Description
Occupational Therapy Evaluation conducted in the home as an outpatient service. Clients selecting this are attesting that they are not under any homecare services currently nor any other occupational therapy services.
This will include at 30-60 minute evaluation in the client’s natural setting to determine overall functional performance in relation to any medical, physical, mental, sensory, or environmental impairment. The evaluation entails a medical history, relevant assessments, and the establishment of a plan of care if warranted to address the nature of complain and/or referral to other professionals based upon the outcome.
